The Sioux Lookout Area Primary Care Team (SLAPCT) works in a team environment to provide wrap-around services and seamless primary care to children, youth, adults and families within both the town of Sioux Lookout and the First Nations communities in the Sioux Lookout region; with total catchment area of 32,000+ people. If you are interested in providing primary care services in both the municipality of Sioux Lookout and directly in the First Nations communities, then this is the job for you! The Primary Care Team, a resource to the First Nations communities in the Sioux Lookout region, brings together a diverse group of health care professionals to work together collaboratively to help meet the individual needs of clients. They deliver care to help clients with specific health care goals and needs, ranging from managing chronic disease, to complex health needs, to helping a client deal with a life challenge that is negatively impacting their health. RESPONSIBILITIES: The Clinical Manager will be responsible for overall clinical leadership and management of all primary care Allied Health Professional staff and contracted health services. The goal of the position is to ensure the Primary Health Care Team produces high quality and timely health services/interventions using culturally appropriate practices and best practice standards of care. . Collaborates with the Director, Primary Care and Human Resources staff to recruit, interview, select, hire and employ staff within the Primary Care Team. . Support program and service evaluation and quality improvement initiatives . Provides supervision, direction and leadership to the Primary Care Team Allied Health disciplines. . Coordinates regular staff meetings, ensuring communication practices are clear and in place. . Provides effective problem solving, and conflict resolution as required. . Administers and monitors adherence to organizational policies and procedures. . Conducts performance appraisals and development of action plans for staff. . Provides ongoing coaching, advice and motivation and any other training needs. . Monitors work plans and operating budgets including variances. . Manages the development of the annual budget, work plans and reports that align with the organizational goals and objectives . Assists with the preparation and submission of funding requests and reports. . Action community concerns and requests appropriately. SKILLS & QUALIFICATIONS . Baccalaureate Degree in Nursing BScN from an accredited University . Current registration with the College of Nurses of Ontario . Minimum of five years' experience in a Nursing setting . Knowledge of First Nations' people, history, culture, health and social issues . Excellent interpersonal, leadership and communication skills . Ability to develop relationships with First Nations people, communities, service stakeholders and health care providers to promote an integrated seamless delivery of services . Ability to function effectively during periods of rapid change . Experience in managing people, program development, governance, budget, resource management, and organizational skills . Experience working with a diverse range of stakeholders, to achieve successful outcomes through relationship-building skills . Willing to travel by various modes and have a valid Driver's License .
